Ontario Student Transcript (OST) 

The Ontario Student Transcript (OST) is an official record that documents a student's academic progress. It is a component of the Ontario Student Record (OSR) and is created and managed in accordance with the

Ministry of Education's OST Manual. Since September 1, 1999, an OST has been established for each student enrolled in an Ontario secondary school course, whether or not the course is being taken for credit.

All graduates receive one diploma and an official transcript at commencement. Additional copies may be requested by completing the form below. The fee is $15.00 per copy.

Note: A completed 'Consent to Release Information to a 3rd Party' form is required if someone other than the adult student is picking up the transcripts.

 

During the school year

Transcript requests are processed by the last secondary school you attended. Please contact the school directly for your transcript requests. Refer to the school directory for school contact information. Payment can be made through School Cash Online

 

Closed schools

Barrie Central Collegiate Institute

Transcripts for former Barrie Central Collegiate Institute students are stored at Barrie North Collegiate Institute. Please contact the school at 705-726-6541.


Midland and Penetanguishene secondary schools

Transcripts for former Midland Secondary School and Penetanguishene Secondary School students are stored at Georgian Bay District Secondary School. Please contact the school at 705-526-7817.

Park Street Collegiate Institute and Orillia District Collegiate Vocational Institute

Transcripts for former Park Street Collegiate Institute and Orillia District Collegiate Vocation Institute students are stored at Orillia Secondary School. Please contact the school at 705-329-4537.

Letters of Education Verification/Attendance


At times, a parent/guardian or adult student may require documentation from their former school to verify attendance or graduation, for government, schools or employment. These requests may take the form of a letter or completed standard forms.
